Open Position Overview We are seeking a qualified Level 1 AP Primary Care professional to provide high-quality primary care services in a clinical healthcare setting. The successful candidate will support patient assessment, diagnosis, treatment planning, health education, and ongoing management of patients while working collaboratively with other healthcare professionals. Key Responsibilities Provide primary care services to patients in accordance with established clinical standards and protocols. Perform patient assessments, obtain medical histories, and evaluate presenting symptoms. Develop appropriate treatment plans based on patient needs and clinical findings. Diagnose and manage common acute and chronic health conditions within scope of practice. Order and interpret appropriate diagnostic tests and laboratory results. Prescribe medications and treatments as permitted by applicable licensure and organizational guidelines. Provide preventive care, health screenings, patient education, and wellness counseling. Monitor patient progress and modify treatment plans when clinically appropriate. Maintain accurate and timely patient documentation and electronic medical records. Coordinate patient care and make referrals to specialists or other healthcare services when needed. Collaborate effectively with physicians, nurses, allied health professionals, and other members of the care team. Follow applicable clinical, regulatory, safety, and infection-control requirements. Maintain patient confidentiality and comply with HIPAA and organizational policies. Required Qualifications Appropriate advanced-practice clinical education and credentials for the assigned role. Current and unrestricted professional license appropriate to practice primary care in the applicable jurisdiction. Relevant primary care/clinical experience. Strong clinical assessment, communication, and patient-care skills. Ability to work effectively as part of a multidisciplinary healthcare team. Proficiency with electronic medical records and clinical documentation. Ability to provide compassionate, patient-centered care. Preferred Qualifications Experience in a primary care or outpatient clinical environment. Experience managing both acute and chronic patient conditions. Experience with preventive medicine and health promotion. Strong patient education and care-coordination skills.
