2996661647-2 Apply for this job now Job Description Hartford Healthcare seeks an Endocrinology Advanced Practice Provider to deliver high-quality, patient-centered care for adults with diabetes and complex hormonal disorders. In this collaborative role, you will perform comprehensive assessments, interpret endocrine labs and imaging, manage medications and technology (including insulin pumps and CGM), and provide ongoing education and support. You'll work closely with endocrinologists and a multidisciplinary team in a culture grounded in compassion, teamwork, and excellence, with access to cutting-edge tools and robust professional development in a leading Connecticut health system. Responsibilities Provide comprehensive evaluation and management of patients with diabetes and complex endocrine disorders Order, interpret, and act on endocrine labs, imaging, and diagnostics Develop individualized, evidence-based care plans and adjust therapies over time Educate patients and families on disease management, medications, and lifestyle changes Collaborate closely with endocrinologists, primary care, and multidisciplinary teams Document all encounters accurately in the EHR and maintain regulatory compliance Participate in quality improvement, clinical protocols, and population health initiatives Precept or mentor staff and contribute to a culture of teamwork and compassion Required Skills Endocrine disorder assessment and management Diabetes management and insulin therapy Thyroid and pituitary disease management Ordering and interpreting labs and imaging Chronic disease care planning Electronic Health Record (EHR) documentation Patient education and counseling Collaborative care with physicians and specialists Procedural skills for endocrine practice (e.g., injections, CGM starts) Evidence-based clinical decision making
