2996670912-2 Apply for this job now Job Description Hartford Healthcare seeks an Endocrinology Advanced Practice Provider to deliver high-quality, patient-centered care within our leading Healthcare & Medical Services network. In collaboration with endocrinologists and an interdisciplinary team, you will evaluate and manage patients with complex endocrine disorders, including diabetes, thyroid, adrenal, and pituitary disease. Responsibilities include comprehensive assessments, diagnostic interpretation, treatment planning, patient education, and care coordination. Responsibilities Provide comprehensive endocrine assessments, diagnosis, and management for adult patients. Manage chronic endocrine conditions such as diabetes, thyroid, adrenal, and pituitary disorders. Order, interpret, and act on lab tests, imaging, and other diagnostic studies. Develop and adjust evidence-based treatment plans and medication regimens. Collaborate closely with endocrinologists, primary care, nursing, and allied health staff. Educate patients and families on disease management, lifestyle modification, and medication use. Document care accurately in the electronic health record and ensure quality and compliance. Participate in case conferences, quality improvement initiatives, and care coordination. Provide occasional inpatient consult support as needed. Precept or mentor students or new APPs as appropriate.
