Provide medical direction and attend to patients in skilled nursing and long-term care facilities in Sulphur, Louisiana, working collaboratively with Nurse Practitioners and Physician Assistants to ensure comprehensive, patient-centered care and clinical oversight. Responsibilities Admit, evaluate, manage, and discharge post-acute and long-term care patients. Provide clinical leadership and oversight for on-site advanced practice clinicians. Coordinate care plans, medications, and transitions with facility staff and families. Complete timely documentation and medical record review using company-provided iPad. Participate in quality improvement and regulatory compliance activities as required. Schedule & Clinical Load Part-time coverage of local skilled nursing facilities (typically 1-2 days per facility per week). No fixed daily start/end times, no overnight shifts, and no on-call responsibilities.

Typical daily caseload:

18-25 visits. Flexible scheduling that supports work-life balance. New graduates who are board-eligible and actively pursuing licensure are encouraged to apply. Qualifications MD or DO with active, unrestricted U.S. medical license or eligibility for licensure. Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine. Strong clinical decision-making, communication, and leadership skills.
