Job Description
Provide medical direction and attend to patients in skilled nursing and long-term care facilities in Sulphur, Louisiana, working collaboratively with Nurse Practitioners and Physician Assistants to ensure comprehensive, patient-centered care and clinical oversight. Responsibilities Admit, evaluate, manage, and discharge post-acute and long-term care patients. Provide clinical leadership and oversight for on-site advanced practice clinicians. Coordinate care plans, medications, and transitions with facility staff and families. Complete timely documentation and medical record review using company-provided iPad. Participate in quality improvement and regulatory compliance activities as required. Schedule & Clinical Load Part-time coverage of local skilled nursing facilities (typically 1-2 days per facility per week). No fixed daily start/end times, no overnight shifts, and no on-call responsibilities.
