Job:
Serve adults 18 and older in our client's Novi office and patients with complex medical needs in homes and senior communities. Bring clinical judgment to the settings where illness, function, family support and the realities of daily life come together.
Contract Type:
Full-Time Direct Hire (W2), part-time considered
Salary :
$230,000/yr-$280,000/yr target full-time cash compensation, with uncapped upside
Location :
Novi, MI Practice broadly. Know your patients. Help shape how care is delivered. A schedule built around a clear role. Home-focused, office-focused and blended schedules are considered. We aim to align the position with your interests while maintaining continuity and dependable coverage. The primary setting, usual workdays, field territory, telephone coverage and cross-coverage are defined before hire. No acute-care hospital rounding is required.
The clinical opportunity:
Care spans adult primary care, geriatrics, dementia, behavioral health and geriatric behavioral medicine, chronic pain, and palliative and hospice coordination. Provide evidence-informed assessment, prevention, chronic disease management, medication optimization and serious-illness conversations. Controlled-substance reduction is individualized and clinically appropriate-not a blanket prescribing policy. The work offers direct, tangible service to patients who may struggle to reach an office, and to families navigating difficult decisions.
Physician partnership and leadership:
Work directly with the physician-owner, with clinical and administrative support and
CGM APRIMA
training. Opportunities include NP/PA collaboration and mentorship, quality improvement, care-model design and program leadership. The scope, time allocation and compensation for supervisory or administrative responsibilities are defined before hire; they are not left as an undefined addition to a visit-based workload.
Compensation and benefits:
A guaranteed minimum with uncapped productivity opportunity. The advertised range is a target for full-time total cash compensation-not a guaranteed base salary or a promise of earnings. The minimum, formula, paid onboarding, schedule and non-visit responsibilities are addressed during recruitment and in the written offer. Medical, dental and vision coverage, retirement plan participation, paid time off and holidays are available to eligible employees under plan terms. Professional liability coverage, CME/licensure support and a field vehicle allowance are included under applicable employment terms. Part-time compensation reflects the agreed schedule and formula.
Responsibilities & Duties:
What you bring: MD or DO; unrestricted Michigan licensure or eligibility; board certification or eligibility in Family Medicine or Internal Medicine. Geriatrics, palliative medicine, hospice, home-based or post-acute experience is valued. Required prescribing authority and payer participation must be in place for assigned work. Strong judgment, accurate documentation, collaboration and reliable follow-through are essential. Field work requires a valid driver's license and reliable insured transportation.
