About Us Root Cause Integrative & Functional Medicine is a growing membership-based practice dedicated to transforming healthcare by integrating traditional and functional medicine. We focus on identifying and addressing the root causes of illness while building meaningful, long-term relationships with our patients. Our model allows providers the time and freedom to practice medicine the way it was intended, with longer appointments, individualized treatment plans, and a strong emphasis on prevention, wellness, and chronic disease reversal. We do not participate with insurance. Our practice operates on a Direct Primary Care/membership model with additional cash-pay services. Position Overview We are seeking a compassionate, patient-centered provider who is passionate about functional and integrative medicine. The ideal candidate enjoys developing deep patient relationships, addressing complex chronic health concerns, and helping patients achieve optimal health through evidence-based, root-cause care. This position may be filled by an: Family Nurse Practitioner (FNP) Physician Assistant (PA-C) Functional medicine experience is preferred but not required for the right candidate who is committed to additional training and professional growth. Responsibilities Conduct comprehensive patient evaluations and wellness assessments Interpret advanced laboratory testing Develop personalized treatment plans Address chronic health conditions using a root-cause approach Provide preventative and lifestyle medicine recommendations Educate and empower patients to take control of their health Collaborate with the clinical team to deliver exceptional patient experiences Maintain accurate and timely medical documentation Why Join Root Cause?
