This role involves assessing patient needs, planning and implementing specific interventions, and evaluating patient progress to aid in recovery and rehabilitation from injury, illness, or disabilities. The successful candidate will customize interventions to improve the patient's ability to perform daily tasks and enhance their quality of life within our compassionate and supportive healthcare framework. Responsibilities Evaluate patient conditions regarding physical and basic mental health along with living skills and determine appropriate therapeutic interventions. Collaborate with interdisciplinary healthcare teams to develop treatment plans that include achievable goals tailored to individual patient needs. Educate patients and family members on therapy techniques that can be continued at home. Document patient progress and adjust treatment plans accordingly, maintaining detailed and confidential records. Required Skills Expertise in therapeutic techniques and diagnosis management.
