Sobe Innovative Rehabilitation is a leading provider of in-home physical, occupational and speech therapy services, dedicated to delivering high-quality care to patients in their homes. We are seeking a Fall Prevention Specialist who brings professionalism, positivity, and reliability to every interaction. This role includes conducting standardized assessments in assisted living facilities and representing Sobe Innovative Rehabilitation during community events and activities. Candidates must feel comfortable working independently while also supporting a collaborative team culture. Primary Responsibilities Conduct standardized assessments with residents in assisted living facilities Lead community based activities that support engagement and wellness Participate in internal meetings as required Attend community events as a representative of the organization Build and maintain positive relationships as a liaison within the community Travel throughout the area to other contracted facilities using reliable transportation is required.
