Job Description

Sobe Innovative Rehabilitation is a leading provider of in-home physical, occupational and speech therapy services, dedicated to delivering high-quality care to patients in their homes. We are seeking a Fall Prevention Specialist who brings professionalism, positivity, and reliability to every interaction. The ideal candidate demonstrates strong integrity, exceptional interpersonal skills, and a genuine dedication to serving others. This role includes conducting standardized assessments in assisted living facilities and representing Sobe Innovative Rehabilitation during community events and activities. Candidates must feel comfortable working independently while also supporting a collaborative team culture. Primary Responsibilities Conduct standardized assessments with residents in assisted living facilities Lead community based activities that support engagement and wellness Participate in internal meetings as required Attend community events as a representative of the organization Build and maintain positive relationships as a liaison within the community Travel throughout the area to other contracted facilities using reliable transportation is required. Mileage reimbursement provided Qualifications Strong time management skills with the ability to prioritize tasks and maintain consistency across multiple locations Proficiency in computer based work, including the use of multiple software platforms, spreadsheets, electronic medical record systems, email communication tools, and mobile technology Ability to follow standardized testing protocols with precision and consistency Ability to maintain confidentiality and adhere to HIPAA guidelines Comfortable engaging in community events and presenting information in a clear professional manner Dependability and punctuality in meeting schedule expectations Why Join Sobe Innovative Rehabilitation You will be part of a compassionate patient focused organization where your work directly impacts the well being comfort and progress of the individuals we serve.
