Job Description

This full-time, 13-week contract opportunity offers experienced registered nurses the chance to provide compassionate, skilled care to patients in their homes. The position is based in the field, visiting patients daily within the assigned territory, and uses the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment tool.
Schedule:
Monday Friday, 8:00 AM 5:00
PM Desired Qualifications:
Active RN license in the state of practice, in good standing At least one year of recent home health nursing experience, with field-based experience strongly preferred Proficiency in Home Care Home Base (HCHB) OASIS assessment and documentation experience Reliable transportation, valid drivers license, and auto insurance
Primary Responsibilities:
Conduct thorough OASIS assessments and develop individualized care plans for each patient Deliver skilled nursing care including wound care, medication administration, IV therapy, and disease management Evaluate patient progress, monitor for changes in status, and maintain open communication with the care team and physicians Educate patients and caregivers regarding disease processes, treatment regimens, and healthy practices Complete accurate, timely documentation within Home Care Home Base (HCHB) Coordinate with other care professionals (PT, OT, medical social work, aides) for holistic patient care Take part in regular case conferences, ongoing education, and staff meetings Follow all regulatory and agency standards in clinical practice and documentation
