This role allows you to deliver personalized care in patients homes and collaborate with a dynamic, professional care team.

Schedule:

13-week contract, with the possibility of extension Full-time hours, Monday through Friday On-call responsibilities are probable

Key Responsibilities:

Deliver high-quality in-home nursing care and skilled clinical interventions Conduct comprehensive patient assessments and complete accurate, timely OASIS documentation Implement physician-ordered care plans through effective collaboration with physicians and caregivers Perform wound care, medication administration, patient education, and chronic disease monitoring Clearly communicate clinical updates and changes in patient status to the interdisciplinary team

What Were Looking For:

Current RN license in good standing At least one year of recent experience as a home health RN Demonstrated OASIS proficiency Strong knowledge of HomeCare HomeBase (preferred) Comfortable managing a full-time, structured daily visit schedule Reliable transportation and a valid drivers license
