This is an excellent opportunity for a dedicated MDS professional who enjoys making an impact through quality resident care, regulatory compliance, and effective interdisciplinary collaboration. The MDS Coordinator will oversee the facility's MDS/RAI process, ensuring accurate and timely assessments while supporting optimal clinical outcomes and reimbursement accuracy. This role works closely with nursing leadership, therapy, social services, dietary, and other departments to ensure resident care plans are comprehensive, compliant, and reflective of individual resident needs. Manage and coordinate the complete MDS assessment process in accordance with CMS RAI guidelines. Ensure accurate and timely completion of Medicare, managed care, quarterly, annual, significant change, and discharge assessments. Collaborate with the interdisciplinary team to develop and maintain effective resident care plans. Review clinical documentation to support quality outcomes and reimbursement accuracy. Monitor regulatory compliance and assist with survey readiness, audits, and quality improvement initiatives. Provide leadership, guidance, and support to promote efficient MDS department operations. Serve as a resource for MDS processes, clinical documentation, and reimbursement-related matters. Qualifications Current California RN or LVN license. Minimum of 2 years of skilled nursing/long-term care experience. Minimum of 1 year MDS/RAI coordination experience. Strong knowledge of CMS regulations, RAI guidelines, and SNF reimbursement processes. Excellent organizational, communication, and leadership skills.
