Job Description

Outstanding Per-Diem Nurse Practitioner needed. Our office is based in Orinda and we are looking for the right person to join our Hospice Care Team. ACE Home Health & Hospice is a locally owned and operated agency. We are growing and are seeking talented and compassionate professionals that want to make a difference in the lives of their clients. The Nurse Practitioner works in collaboration with the Medical Director, patient physicians, and team members, in an active practice to deliver, and support appropriate clinical care under Hospice federal and state conditions of participation, and all Ace Home Health and Hospice policies and procedures to ensure excellence involving all aspects of the patient's clinical care.
PRIMARY RESPONSIBILITIES - CLINICAL
Utilizes approved clinical guidelines and evidence-based protocols when providing care. Performs comprehensive and focused evaluation of patients, including the required face to face assessment , as indicated. Supports timely admission into the Hospice and Palliative Programs, removing access barriers when identified or assists with the Admission scheduling and assignment when necessary, including the solicitation, and gathering of patient medical record prior to patients' admission for the Hospice Medical Director's evaluation, and assessment for patient appropriateness. Consults and collaborates with Medical Director or Hospice Physician designee, referring, and Attending Physicians, and designee as needed, providing written reports on patient contracts, and communications upon request. Provides accurate documentation and maintains patient records to support sound medical practice, and coding for reimbursement for services provided in a timely manner when/if necessary or appropriate.
