San Gabriel Convalescent Center is a 129 -Bed Skilled Nursing & Subacute Facility. Dedicated to meeting the physical, psychosocial and emotional needs of our residents. SUMMARY:

We are looking for an experienced RN. The RN should be able to provide prescribed medical treatment and personal care services to residents and assists in providing a clean, safe, dignified, happy and healthy environment for residents by performing the following duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES

include the following: Complies with the facility's attendance policy requiring regular, reliable, and punctual attendance. Assesses, communicates and reports all changes in the resident's condition verbally and by written report to other staff, other shifts, family members, physician and others as indicated by policies and procedures and state/federal regulations Accompanies physicians who are visiting residents and accurately records data from that visit in a timely, accurate and factual manner. Monitors resident status closely by assessment and review of the medical record and notifies the physician as indicated according to the Company's policies and procedures and by current standards of care and practice. Communicates with doctors about prescriptions. Any medication orders given in writing are to be implemented immediately. All changes in medications are also to be documented in M.A.R. and "Centrally Stored Medication" binder. Meet and greet clients, family members and visitors. Completes incident reports within 24 hours of incident and insures med tech completes incident reports within 24 hours. Reports are handed to Administrator. Follows resident care plan for ALWPP residents including measuring and recording patients' vital signs, such as height, weight, temperature, blood pressure, pulse and respiration, as written. Reads daily logbook and nursing notes. Checks on any problems reported on previous shifts. If any resident is sent to the hospital, calls and checks on their progress. Creates, maintains and distributes department schedule. Assessments are to be noted in resident file if there are any "changes in condition. Maintains the comfort, privacy, and dignity of each resident in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring environment. Conducts staff in-services and on-going shadow training, New Hire Training as well as providing orientation for new department employees. Attends, participates training, educational activities, and staff meetings. Maintains records neatly and orderly in designated filing areas. Provides basic patient care and treatments, such as taking temperatures or blood pressures, dressing wounds, treating skin issues, or maintaining and monitoring catheterizations and colostomies, injections and blood sugar checks. Observe residents, documenting and reporting changes in resident's conditions, such as adverse reactions to medication or treatment, and taking any necessary action. Assembles and uses equipment such as catheters and oxygen suppliers. Collects specimen samples from residents as ordered by doctor. Ensures that all staff members comply with company policies and procedures and effectively communicates changes to the department staff Assists in maintaining a safe, neat, and clean environment; Notifies Administration of any unusual situations or unsafe environment. Responds to inquiries or requests from residents, visitors, other personnel, etc. within given time frames and within established policy, and without breaking confidentiality. Maintains resident confidentiality; treats residents with kindness, dignity, and respect; knows and complies with Residents' Bill of Rights; and promptly reports all resident complaints, accidents, and incidents to Administrator and/or Director of Nurses Other assignments may be added as directed by the Administrator and/or the Director of Nurses. Rate can be negotiated based on availability and experience!
