2996666681-2 Apply for this job now Job Description Hartford Healthcare seeks a compassionate Revisit Nurse to provide follow-up nursing care to patients across our network. In this role, you will conduct reassessments, update care plans, monitor symptoms, and ensure safe transitions in home or ambulatory settings. You'll document in the EHR, coordinate with an interdisciplinary team, and deliver patient and family education. Responsibilities Conduct follow-up (revisit) nursing assessments for patients in home or ambulatory settings Review and update individualized care plans based on patient progress and provider orders Document all visits accurately in the EHR and communicate changes to the care team Monitor vital signs, symptoms, and treatment adherence, escalating concerns as needed Provide patient and family education on disease management, medications, and safety Collaborate with physicians, therapists, and case managers to coordinate services Ensure compliance with Hartford Healthcare clinical standards and regulatory requirements Participate in quality improvement initiatives and case conferences Support a culture of compassion, inclusion, and teamwork Use evidence-based practices and technology to optimize patient outcomes Required Skills Patient assessment Care plan evaluation Electronic Health Records (EHR) documentation Medication reconciliation Wound and post-procedure assessment Care coordination Vital signs monitoring Infection control practices Health education and discharge teaching Clinical decision-making
