Care At Home - Nurse Practitioner - Litchfield County, CT Optum 401(k) United States, Connecticut, Hartford Aug 03, 2026 Litchfield & Waterbury, CT Optum Home & Community Care is seeking a Nurse Practitioner to provide in-home care for high-risk patients in Litchfield County, Connecticut. This role focuses on delivering comprehensive, longitudinal care in the patient's home while collaborating with an interdisciplinary care team to improve outcomes and reduce barriers to care. Position Highlights $40,000 Student Loan Repayment or $20,000 Sign-On Bonus No nights, weekends, or holidays Daily travel within the assigned territory Provide in-home assessments and ongoing care management Manage acute and chronic conditions, including post-acute rehabilitation needs Conduct advanced care planning and patient education Collaborate with interdisciplinary care teams Opportunity to care for high-risk, medically complex patients in their homes Required Qualifications
For NPs:
Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations Active and unrestricted driver's license In addition to a driver's license, access to reliable transportation Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) Ability to gain a collaborative practice agreement, if applicable in your state Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
