2996669488-2 Apply for this job now Job Description Hartford Healthcare seeks a compassionate Hospice RN to provide expert end-of-life care across our Connecticut network. In this field-based role, you'll manage a caseload of hospice patients, performing assessments, creating and updating care plans, and delivering pain and symptom management in homes and facilities. You'll educate and support families, coordinate with an interdisciplinary team, and document care in the EMR. Responsibilities Provide compassionate end-of-life nursing care to hospice patients in homes or facilities Perform comprehensive patient assessments and update individualized care plans Administer medications, treatments, and pain/symptom management per provider orders Educate patients and families on disease process, comfort measures, and care techniques Coordinate care with physicians, social workers, chaplains, and other team members Document all care, assessments, and communications accurately in the EMRParticipate in on-call rotation and respond to urgent patient needs Support families through emotional, cultural, and spiritual aspects of end-of-life care Ensure compliance with hospice, safety, and infection control standards Contribute to quality improvement and professional development activities Required Skills Hospice nursing Pain and symptom management Palliative care protocols IV and medication administration Wound care Electronic Medical Records (EMR) documentation Care planning and case management Patient and family education Interdisciplinary team collaboration Infection control and safety compliance
