The ideal candidate is a compassionate, organized healthcare professional who enjoys patient education, community health, and helping individuals take an active role in improving their health. What You'll Do Support diabetes prevention and wellness initiatives within the community. Provide health education and guidance related to diabetes prevention, healthy lifestyle choices, nutrition, and physical activity. Work with individuals to encourage positive and sustainable behavior changes. Assist with participant outreach, engagement, and follow-up. Help monitor participant progress and maintain accurate program documentation. Provide education and resources to individuals regarding diabetes risk reduction. Collaborate with healthcare professionals, community organizations, and program staff. Maintain confidentiality and follow applicable healthcare and program requirements. Participate in meetings, training, and other program activities as needed. Complete other duties related to diabetes prevention and community health initiatives. What We're Looking For Registered Nurse (RN) with an active nursing license. Experience in public health, community health, health education, diabetes prevention, or a related healthcare setting preferred. Strong communication and interpersonal skills. Ability to educate and engage individuals from diverse backgrounds. Strong organizational and documentation skills. Ability to work independently while also collaborating effectively with a team. Comfortable working with community members and healthcare partners.
