Schedule & Setting Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Field-baseddaily travel to patient homes within your assigned territory What Youll Do Conduct comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management Monitor and evaluate patient progress, identifying and communicating changes in condition to the care team and physicians Educate patients and families about disease processes, treatment plans, medications, and preventive care Maintain meticulous documentation using Home Care Home Base (HCHB) Collaborate with an interdisciplinary team including therapists, medical social workers, and aides Participate in case conferences, staff meetings, and continuing education to promote best practices Uphold all regulatory and organizational standards for documentation and patient care Qualifications Active RN license in the state of practice, in good standing At least 1 year of recent field-based Home Health nursing experience required Proficient in Home Care Home Base (HCHB) and OASIS Reliable transportation, valid drivers license, and auto insurance required Benefits Weekly pay Comprehensive Medical, Dental, and Vision Insurance options 401(k) retirement plan options 24/7 support from a dedicated team Mileage reimbursement Opportunity for contract extensions and renewals Showcase your clinical skills and compassion by leading patient care in the community.
