Key Responsibilities:

Perform in-home annual risk assessments for Medicare Advantage members within Leon County (locations may include Tallahassee, Woodville, and Midway) Review and evaluate patients current and prior medical and medication histories Provide clear education to patients regarding their conditions and current medication regimens Identify and document any care gaps, offering actionable recommendations to help optimize patient health outcomes Focus exclusively on assessmentno treatment planning or medication prescribing required

Qualifications:
