PM Key Qualifications:

Active RN license, in good standing, in the state of practice Minimum of one year of recent Home Health experience (field experience strongly preferred) Proficiency in Home Care Home Base (HCHB) Experience with OASIS assessments and documentation required Reliable transportation, valid driver's license, and current auto insurance

Primary Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management Regularly evaluate patient progress, promptly reporting status changes to the care team and physicians Provide thorough patient and family education on disease processes, medications, and preventive care strategies Maintain detailed, timely documentation utilizing HCHB Collaborate with a multidisciplinary teamincluding therapists, social workers, and home health aidesto provide coordinated, holistic care Attend case conferences, participate in staff meetings, and engage in ongoing professional development Ensure compliance with all agency and regulatory standards
