RN/Wound Care Nurse Position Summary The RN Wound Care Nurse is responsible for coordinating, providing, and monitoring wound prevention and treatment services for residents within the skilled nursing facility. This position performs comprehensive skin and wound assessments, implements physician-ordered treatment plans, monitors healing progress, maintains accurate clinical documentation, and collaborates with the interdisciplinary team to promote optimal resident outcomes. The RN Wound Care Nurse also serves as a clinical resource to nursing staff by providing education regarding wound prevention, pressure injury management, skin integrity, treatment techniques, and facility protocols. All duties are performed in accordance with professional nursing standards, facility policies, physician orders, and applicable federal and Florida requirements.

Essential Duties and Responsibilities Perform comprehensive skin and wound assessments upon admission, readmission, change in condition, and as clinically indicated. Assess and monitor pressure injuries, surgical wounds, diabetic ulcers, venous and arterial ulcers, skin tears, and other alterations in skin integrity. Measure, evaluate, document, and photograph wounds in accordance with facility policy and applicable requirements. Develop and implement individualized wound care interventions in collaboration with physicians, advanced practice providers, nursing staff, therapy, dietary services, and other members of the interdisciplinary team. Perform wound treatments and dressing changes in accordance with physician orders, accepted standards of nursing practice, and facility protocols. Monitor residents' response to treatment and identify changes in wound condition, healing progress, or potential complications. Promptly communicate significant changes or concerns to the physician or appropriate provider and Director of Nursing. Participate in the development and revision of resident care plans related to wounds, skin integrity, pressure injury prevention, nutrition, positioning, and other identified risk factors. Identify residents at risk for skin breakdown and ensure appropriate preventive interventions are implemented. Complete and monitor pressure injury risk assessments and other applicable clinical assessments. Conduct routine skin rounds and wound rounds. Coordinate appointments and recommendations from wound care physicians, specialists, and outside wound clinics when applicable. Ensure treatment orders are current, accurately transcribed, and properly implemented. Maintain timely, complete, and accurate documentation within the resident's medical record. Monitor wound care supplies and assist with ensuring appropriate products and equipment are available. Evaluate the effectiveness and appropriate use of pressure-relieving mattresses, cushions, positioning devices, and other preventive equipment. Educate nursing staff regarding wound prevention, treatment procedures, documentation requirements, and changes in resident treatment plans. Provide resident and family education regarding wound care, prevention strategies, and treatment goals as appropriate. Participate in interdisciplinary care plan meetings, clinical meetings, quality assurance activities, and other meetings as assigned. Assist with investigation, tracking, trending, and analysis of facility-acquired pressure injuries and other skin integrity concerns. Support Quality Assurance and Performance Improvement (QAPI) initiatives related to wound prevention and management. Maintain infection prevention and control practices during all wound care procedures. Follow appropriate hand hygiene, PPE, clean/aseptic technique, and disposal procedures. Maintain resident privacy, dignity, confidentiality, and rights at all times. Report suspected abuse, neglect, exploitation, injuries of unknown origin, or other reportable concerns in accordance with facility policy and applicable requirements. Assist nursing staff with resident care and other clinical responsibilities when necessary and within the scope of RN practice. Perform additional duties as assigned by the Director of Nursing or Administrator. Qualifications Current, active Registered Nurse (RN) license in the State of Florida or a valid multistate license permitting practice in Florida. Graduate of an accredited school of professional nursing. Current CPR/BLS certification as required by facility policy. Previous wound care experience in a skilled nursing, long-term care, rehabilitation, hospital, home health, or similar healthcare setting preferred. Previous skilled nursing or long-term care experience preferred. Wound care certification such as WCC, WOCN-related certification, or equivalent is preferred but not required unless specified by the facility. Knowledge of wound assessment, pressure injury prevention, treatment modalities, infection prevention, and documentation standards. Knowledge of federal and state requirements applicable to nursing services and resident care in a skilled nursing facility. Ability to recognize changes in resident condition and communicate effectively with physicians, providers, residents, families, and interdisciplinary team members. Strong clinical assessment, organizational, documentation, and time-management skills. Ability to educate and provide clinical guidance to nursing staff. Must successfully complete all background screening, health screening, and other employment requirements applicable to the position and facility. Knowledge, Skills, and Abilities The RN Wound Care Nurse should demonstrate: Strong knowledge of skin and wound assessment. Understanding of pressure injury staging and prevention. Knowledge of common wound care products and treatment modalities. Ability to evaluate wound progression and identify signs of infection or deterioration. Strong attention to clinical documentation and regulatory compliance. Effective communication and interdisciplinary collaboration skills. Duties may require:
Standing and walking for extended periods. Bending, reaching, kneeling, and positioning residents during assessments and treatments. Assisting with lifting, transferring, repositioning, or moving residents in accordance with facility procedures and safe patient-handling practices. Using appropriate mechanical lifting and transfer equipment when indicated. Performing repetitive hand movements associated with wound treatment and clinical documentation. Working in an environment with potential exposure to blood, bodily fluids, infectious materials, and other healthcare-related occupational hazards. Infection Prevention and Safety The RN Wound Care Nurse is expected to follow all facility infection prevention and safety policies, including Standard Precautions, transmission-based precautions when applicable, appropriate use of personal protective equipment, hand hygiene, wound care technique, and proper handling and disposal of contaminated materials.
