Job Summary We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a vital member of our in-home healthcare services. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a passion for providing compassionate care. In this role, you will deliver personalized nursing services, manage complex medical treatments, and collaborate with families and healthcare teams to ensure optimal patient outcomes. Responsibilities Provide comprehensive nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring. Perform patient assessments and develop individualized care plans tailored to each patient's unique medical needs. Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate documentation of patient care activities. Collaborate closely with physicians, case managers, and families to coordinate care plans and ensure continuity of services. Manage acute pain and chronic condition management while adhering to best practices in infection control and safety protocols. Assist with specialized procedures including dialysis, cardiology-related interventions like catheterization, and respiratory support such as ventilator management. Educate patients and caregivers on health coaching, medication administration, and post-discharge care instructions to promote independence and safety. Experience Candidates should demonstrate prior experience working in diverse healthcare settings such as hospital medicine, critical care units including ICU or Level I Trauma Centers, emergency rooms, or outpatient clinics. Experience working with individuals with disabilities or in geriatrics—including memory care or assisted living environments—is highly valued. Familiarity with EMR/EHR systems like Athenahealth or eClinicalWorks is preferred. A background in medical-surgical nursing combined with critical care experience will support success in this role.
