Monday Friday 8:00 AM 5:00

PM Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress and promptly communicate any status changes to the care team and physicians. Educate patients and families on disease management, medications, and preventive strategies. Maintain accurate, timely electronic records using Home Care Home Base (HCHB). Collaborate with physical therapists, occupational therapists, social workers, and aides for coordinated care. Participate in case conferences, team meetings, and professional development activities. Adhere to regulatory and agency standards for compliance and quality.

Qualifications:

Current RN License in good standing within the state of practice At least 1 year of recent field-based Home Health experience Proficient in HCHB electronic documentation Experience with OASIS assessments required Reliable transportation, valid drivers license, and proof of auto insurance
