This full-time, 13-week contract role involves delivering high-quality nursing care to individuals in their homes, supporting them in managing health conditions and achieving optimal outcomes. You will serve as a trusted clinical partner, coordinating tailored care plans and promoting patient independence.

Schedule/Hours:

Monday Friday, 8:00 AM 5:00 PM Expect a caseload of 2532 points per week (typically 35 patient visits per day)

Essential Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, document with Home Care Home Base (HCHB), and communicate effectively with physicians and the care team Provide education to patients and families regarding disease processes, treatment plans, and medication adherence Collaborate with interdisciplinary team members to ensure coordinated and high-quality care Attend case conferences and staff meetings, and adhere to regulatory and documentation standards

Required Skills & Experience:

Active RN license in good standing in the state of practice Minimum one year of recent home health field experience Proficient with Home Care Home Base (HCHB) and OASIS assessments Reliable transportation, valid drivers license, and auto insurance Excellent assessment, organizational, and communication abilities Comfortable managing a flexible, field-based schedule

Preferred Qualifications:

BSN degree Experience handling a caseload of 2532 points per week Familiarity with Medicare guidelines and home health regulations
