Job Description

Deliver exceptional patient care as a Nurse Practitioner in a dynamic per diem home health role within Bonner County. You'll make a meaningful impact by traveling to member homes across Sandpoint, Ponderay, Priest River, and nearby areas, providing high-value risk assessments for Medicare Advantage plan members. As a vital part of preventative health, your expertise will support members ongoing well-being through focused, 45-minute evaluations.

Flexible shiftschoose any day, Monday through Sunday Set your own hours and create a schedule that fits your lifestyle Minimum weekly or bi-weekly commitments available
Key Responsibilities:
Conduct annual health risk assessments in patient homes as part of Medicare Advantage initiatives Review and assess prior/current medical history, chronic or acute medical conditions, and current medication regimens Educate members about their diagnoses, treatment goals, and medication usage Identify care gaps and offer actionable recommendations to improve patient outcomes No prescribing or treatment plan development requiredfocus is on assessment, education, and bridging care Spend approximately 45 minutes per evaluation
Qualifications:
Active Nurse Practitioner license in Idaho Board certified through AANP or
ANCC CPR
certification (BLS) 6+ months of current NP experience preferred MSN from an accredited nursing program Your commitment and expertise will be rewarded with $80-$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
