A per diem opportunity is available for a Nurse Practitioner specializing in Home Health assessments in Shoshone County, ID. In this role, you will conduct comprehensive risk assessments for Medicare Advantage plan members, supporting high-quality care in various areas including Kellogg, Pinehurst, and Osburn. Each assessment requires approximately 45 minutes and could involve either an annual evaluation or a post-discharge visit, depending on the patients needs.

Schedule:

Flexible shifts offered seven days a week Ability to create your own schedule Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage health plan members Review comprehensive medical, surgical, and medication histories with patients Educate patients about their chronic or acute conditions, and proper medication use Identify care gaps and make suggestions for improvement

Note:

No development of treatment plans or prescribing responsibilities

Qualifications:

Active Nurse Practitioner license in Idaho Current BLS certification Board Certification through AANP or ANCC required At least 6 months of experience as a Nurse Practitioner preferred Graduated with an MSN from an accredited nursing program The compensation for this per diem position ranges from $80 to $90 for each evaluation, with additional stipends provided for mileage and administrative time.
