Schedule:

Flexible shifts available seven days a week Create your own schedule Commitments can be arranged weekly or bi-weekly

Key Responsibilities:

Complete thorough annual home health risk assessments for Medicare Advantage plan members in their own homes Review and evaluate past and current medical history, including health conditions and medication usage Provide patients with education regarding their health and prescribed treatments Identify gaps in care and offer evidence-based recommendations to address these areas

Note:

Treatment plans, new orders, or prescriptions are not required as part of these visits

Preferred Qualifications:

Valid Nurse Practitioner license for Illinois Board certification through AANP or ANCC Current BLS (CPR) certification Master of Science in Nursing (MSN) from an accredited program At least six months of experience as a Nurse Practitioner is preferred
