Key Responsibilities:

Complete detailed annual home health risk assessments for Medicare Advantage members in their homes Review and assess prior and current medical histories, active health conditions, and medication usage Educate members on their medical conditions, medications, and overall health maintenance Identify gaps in care and offer practical recommendations for improved health outcomes No prescribing or treatment plan development requiredfocus is solely on evaluation and patient education

Desired Qualifications:

Active Nurse Practitioner license in Illinois Current Basic Life Support (BLS/CPR) certification Board Certification from AANP or ANCC At least six months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution You will receive competitive compensation, earning $80-$90 per completed evaluation in addition to a mileage stipend and coverage for administrative time.
