Overview Nexum HC is seeking a compassionate and dedicated LCSW, LCPC or LMFT to join our team providing psychotherapy services within a skilled nursing and rehabilitation facility in Oak Park, Illinois. This position offers the opportunity to make a meaningful impact in the lives of older adults by providing high-quality mental health services in a supportive healthcare environment. The ideal candidate will be committed to delivering compassionate, person-centered care while collaborating with facility staff and interdisciplinary teams. Responsibilities Conduct comprehensive assessments and diagnostic evaluations to identify mental health needs Develop and implement individualized treatment plans based on client goals and clinical best practices Provide individual and group psychotherapy utilizing evidence-based therapeutic interventions Facilitate group therapy sessions as appropriate Maintain accurate and timely clinical documentation in compliance with legal and ethical standards Collaborate with nursing staff, physicians, social services, and other healthcare professionals to coordinate care Support residents experiencing depression, anxiety, grief, adjustment concerns, and other behavioral health needs Promote emotional wellness and improve quality of life for residents Qualifications Master's degree in Social Work, Counseling, Psychology, or a related field Active Illinois licensure as an LCSW, LCPC or LMFT Strong clinical assessment, treatment planning, and documentation skills Excellent communication and interpersonal skills Ability to work independently while collaborating with a multidisciplinary team Experience working in nursing homes, assisted living, rehabilitation, or healthcare settings preferred but not required Experience providing individual and group therapy preferred Why Join Nexum HC?
