Job Description

Job Summary We are seeking a highly motivated and experienced nurse to join our team as a nurse case manager and key advocate for our patients. This role will function under the supervision of the Manager of Longitudinal/Transitional Chronic Care Management Programming. The role requires strong nursing and case management skills and the ability to collaborate with various departments and staff to achieve department and company goals. Responsibilities Below are the roles and responsibilities that the Nurse Case Manager is expected to fulfill.
  • Develop, implement, and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams.
  • Act as the primary point of contact and advocate for patients and their families, facilitating access to medical providers and community resources.
  • Provide essential health education to patients and families to improve understanding and management of chronic conditions.
  • Conduct thorough assessments to identify unmet health and social needs.
  • Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication.
  • Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status.
  • Ensure effective communication across all levels of the organization to facilitate teamwork and collaboration. Skills
  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of Illinois.
  • Experience in triage, post-acute or long-term care settings within the last 5 years.
  • Self-motivated and able to work both independently and in collaboration with others.
  • Strong analytical skills with the ability to interpret data and make informed decisions.
  • Ability to communicate proficiency through technology and maintain accurate notes and records.
  • Ability to maintain confidentiality of all medical, financial, and legal information.
  • Exceptional communication skills, both verbal and written.
  • Ability to thrive in a fast-paced environment while managing multiple priorities.

Pay:
$34.00 - $36.00 per hour
