A full-time opportunity is available for a Registered Nurse with expertise in Home Health, offering a 13-week contract. This role focuses on field-based care, providing compassionate nursing services directly in patients' homes. You'll play a pivotal role in developing and delivering individualized plans of care, ensuring your patients receive the highest standard of clinical support.

Schedule & Case Expectations:

Monday through Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (typically 35 patient visits per day)

Requirements:

Current, active RN license in good standing At least 1 year of recent Home Health nursing experience (field experience strongly preferred) Demonstrated proficiency with Home Care Home Base (HCHB) OASIS assessment experience required Valid drivers license, reliable vehicle, and auto insurance

Key Responsibilities:

Complete comprehensive OASIS assessments and develop tailored care plans Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, identify status changes, and update the interdisciplinary team Educate patients and families on care management, medications, and preventative strategies Ensure precise, timely documentation in HCHB Coordinate with physical therapists, occupational therapists, social workers, and aides for holistic patient care Attend case conferences, meetings, and continuing education as needed Uphold all regulatory and agency standards for home health services
