Key Responsibilities:

Perform comprehensive annual home health risk assessments in patient homes Review and document current and prior medical history, ongoing conditions, and medication usage Educate members on their medical conditions and medications Recognize gaps in care and recommend appropriate follow-up actions No requirement for developing treatment plans or prescribing medications

Qualifications:
