Job Description

Make a meaningful impact in community healthcare by leveraging your expertise in home health as a Nurse Practitioner. This Per Diem opportunity focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Fountain County. With visits in areas such as Attica, Covington, and Veedersburg, you play a crucial role in improving patient outcomes by providing quality in-home assessments.
Schedule:
Flexible shifts available Monday through Sunday Create your own schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform comprehensive annual risk assessments in patient homes Evaluate and document previous/current medical history, health conditions, and medication regimes Educate members on their health conditions and prescribed medications Identify gaps in care and offer recommendations to close those gaps No requirement for creating treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Indiana Valid CPR (BLS) certification Board certification through AANP or ANCC Minimum of 6 months' Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution This role offers competitive compensation of $80-$90 per evaluation, along with a mileage stipend and pay for administrative time.
