This per diem opportunity allows for maximum flexibility, enabling you to design your work schedule and choose shifts between Monday and Sunday. Youll be instrumental in supporting the health and wellness of patients residing in Galveston, Logansport, Walton, and surrounding areas through comprehensive home-based evaluations.

Schedule:

Key Responsibilities:

Complete annual risk assessments within patients homes, most lasting around 45 minutes Evaluate comprehensive medical histories, current health status, and medication use Educate patients on both conditions and medication management Identify care gaps and make actionable recommendations for improved outcomes No requirement for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR (BLS) certification Board-certified through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of current Nurse Practitioner experience preferred
