Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members during home visits Review and evaluate prior and current medical history, ongoing health conditions, and medication regimens Provide targeted education regarding chronic conditions and medication usage to patients and their families Identify care gaps and recommend actions to optimize patient health outcomes Visits may include annual health assessments or post-discharge evaluations; there is no requirement for prescribing medication or developing treatment plans

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board certification through AANP or ANCC Minimum of 6 months of current NP experience preferred Master of Science in Nursing (MSN) from an accredited nursing program This home health role is compensated at $80$90 per evaluation, plus a mileage stipend and additional administrative time, ensuring your expertise and commitment are recognized and rewarded.
