Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review and document prior and current medical history, relevant health conditions, and medication usage Educate patients on their chronic medical conditions and proper medication adherence Identify care gaps and offer actionable recommendations to help improve outcomes No requirement to formulate treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Indiana Board certification through AANP or

ANCC MSN
