Conduct thorough home health risk assessments, averaging 45 minutes per visit Evaluate patients medical and medication histories, as well as current medical conditions Complete annual health assessments and occasional post-discharge visits Educate patients on their medical conditions and prescribed medications Identify care gaps and provide clear recommendations for follow-up No requirement to develop treatment plans or prescribe medication

Qualifications:

Active Nurse Practitioner license in IN Current BLS (CPR) certification Board Certification via AANP or

ANCC MSN

degree from an accredited nursing school At least six months of recent experience as a Nurse Practitioner preferred Candidates may be scheduled for visits in Lawrenceburg, Greendale, Aurora, or other areas throughout Dearborn County, ensuring variety and a meaningful connection with the local community.
