Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage enrollees Evaluate medical history, chronic conditions, and current medication regimens Deliver clear patient education on diagnoses and medications Identify care gaps and offer actionable recommendations Ensure documentation is complete and accurate after each assessment Conduct each visit within an average timeframe of 45 minutes Focus on patient-centered care and accurate reporting; no follow-up prescribing or plan development obligations

Qualifications:

Active Nurse Practitioner License in Indiana Current CPR certification (BLS) National board certification (AANP or ANCC) MSN from an accredited nursing program At least 6 months of recent NP experience preferred Excellent communication and organizational abilities This position offers $80-$90 per evaluation, a mileage stipend, and compensation for administrative time, providing a rewarding blend of autonomy and competitive earning potential.
