This opportunity is ideal for a Nurse Practitioner seeking a per diem position focused on home health risk assessments for Medicare Advantage members within Fountain County, IN. In this role, you will conduct comprehensive evaluations in patients homes located in areas including Attica, Covington, and Veedersburg. Each assessment generally takes about 45 minutes and may include either annual health exams or post-discharge follow-ups.

Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review and evaluate patients past and current medical history, chronic conditions, and medications Deliver patient education related to medical conditions and medication management Identify care gaps and recommend appropriate next steps for the patient and care team The role does not require creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current BLS (CPR) certification Board Certification through AANP or ANCC Minimum of 6 months of nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution Compensation for this role includes $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
