Deliver exceptional care and make a meaningful impact in your community as a Home Health Registered Nurse (RN). This full-time, 13-week contract offers you the opportunity to provide high-quality, patient-centered care directly in the homes of those who rely on your clinical expertise.

Schedule:

Monday Friday, 8:00 AM 5:00

PM Key Responsibilities:

Conduct comprehensive OASIS assessments to develop individualized care plans for each patient. Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management. Monitor patient progress, identify changes in condition, and promptly update the care team and physicians. Educate patients and their families on medications, disease processes, preventive strategies, and overall care management. Maintain accurate and timely electronic records within Home Care Home Base (HCHB). Coordinate care and communicate effectively with an interdisciplinary team, including therapists, social workers, and home health aides. Participate in case conferences, staff meetings, and ongoing training. Ensure compliance with agency policies and regulatory standards.

Required Qualifications:

Active RN license in good standing for the state of practice Minimum 1 year of recent, hands-on Home Health RN experience (field experience highly preferred) Proficiency with OASIS and Home Care Home Base (HCHB) Demonstrated strong clinical, communication, and organizational skills Reliable transportation, valid drivers license, and current auto insurance Independent, self-directed work ethic with excellent time management

Preferred:

BSN Experience with a 2532 weekly point caseload Familiarity with Medicare/home health compliance
