Job Description

Take on a dynamic role delivering skilled nursing care in a field-based home health setting. This full-time, 13-week contract position offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, where youll make a meaningful impact with every patient visit. This opportunity focuses on providing quality care while balancing manageable caseload expectationstypically 35 patient visits per day, aiming for 30 points per week.

PM Key Qualifications:
Active RN license in the state of practice (in good standing) At least 1 year of recent home health experience (field experience preferred) Proficiency in Home Care Home Base (HCHB) electronic documentation OASIS assessment tool experience is required Reliable transportation, valid drivers license, and current auto insurance
Primary Responsibilities:
Perform comprehensive OASIS assessments and develop individualized patient care plans Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management Monitor patient conditions, evaluate progress, and communicate updates to physicians and care teams Educate patients and families regarding treatment plans, medications, and preventive care Document all assessments and care provided in HCHB in a timely, accurate manner Collaborate closely with therapists, medical social workers, and aides for coordinated, patient-centered care Attend case conferences, staff meetings, and participate in continuing education opportunities Adhere to all regulatory and agency policies regarding patient care and documentation
