Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members throughout Calcasieu County, including areas such as Lake Charles, Sulphur, and DeQuincy Evaluate patients' prior and current medical history, assess ongoing medical conditions, and review medication usage Offer educational guidance on various health conditions and prescribed treatments Identify gaps in care and provide recommendations to enhance patient outcomes Focus on assessment and educationno requirements to prescribe medications or develop treatment plans

Qualifications:

Active Nurse Practitioner license in Louisiana MSN degree from an accredited school of nursing Board certification via AANP or ANCC Current BLS (CPR) certification At least 6 months of recent NP experience is preferred This role provides an excellent earning potential, with payments of $80$90 per completed evaluation plus a mileage stipend and compensation for administrative time.
