A fulfilling opportunity awaits experienced Registered Nurses seeking a field-based role providing high-quality home health care. This is a full-time, 13-week contract position, with a weekday schedule that allows weekends off.

Monday Friday, 8:00 AM 5:00

PM Role Overview:

Deliver skilled nursing care to patients in their homes, managing a caseload of approximately 35 visits per day (30 points per week). This position involves daily travel within the assigned territory, utilizing a mileage reimbursement benefit. Comprehensive patient assessments and documentation will be completed in the Home Care Home Base (HCHB) system, with OASIS as the primary assessment tool.

Key Responsibilities:

Conduct OASIS assessments and develop individualized care plans.

Provide skilled nursing interventions:

wound care, IV therapy, chronic disease management, and medication administration. Monitor and evaluate patient progress, collaborating with physicians and the care team. Educate patients and families on treatment plans, disease management, and preventive care. Maintain thorough documentation in HCHB. Work closely with interdisciplinary team members to coordinate comprehensive care. Attend case conferences, staff meetings, and participate in ongoing education. Adhere to regulatory and agency standards ensuring safe, compliant care.

Qualifications:

Current, active RN license in good standing for the state of practice. At least 1 year of recent home health experience; field experience is highly valued. Proficient in HCHB and experienced with OASIS. Reliable vehicle, valid drivers license, and up-to-date auto insurance.
