PM Key Details:

Field-based position, visiting patients in their homes within an assigned territory Caseload of approximately 35 patient visits per day, totaling 30 points per week Utilizes Home Care Home Base (HCHB) electronic documentation system Daily travel with mileage reimbursement provided

Responsibilities:

Conduct thorough OASIS assessments to develop personalized care plans Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management Continuously assess patient progress, report changes, and coordinate with interdisciplinary team members Educate patients and families about treatment plans, medications, and health maintenance Maintain precise, timely documentation in HCHB Attend case conferences, staff meetings, and continuing education sessions as required Uphold all professional, regulatory, and agency standards

Qualifications:
