Job Description
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. Well established hospice agency has a need for an experienced Hospice RN to see patients mostly in facitlities located in the Quincy/Franklin/Mansfield MA areas. Requires minimum 1 year of Hospice RN experience with a Medicare certified agency. Primary nurse provides quality nursing care to identified Hospice patients. The scope of the primary Hospice RN includes direct patient care, coordination of patient and patient advocacy. Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.
