Yes, reimbursed between patient visits Coverage Area Boston and surrounding regions, including but not limited to: South Boston Dorchester Back Bay Roxbury Beacon Hill South End Position Summary CareTenders of Needham is seeking an experienced and compassionate Travel Registered Nurse (RN) to provide exceptional home health services to patients throughout Boston and surrounding communities. The RN will be responsible for comprehensive patient assessments, development and management of care plans, coordination of care with physicians and interdisciplinary team members, and oversight of home health support staff. This role requires strong clinical judgment, excellent documentation skills, and the ability to work independently in a fast-paced home health environment. Key Responsibilities Patient Assessment & Care Planning Perform comprehensive nursing assessments and develop individualized plans of care in collaboration with physicians and other healthcare professionals. Initiate, implement, evaluate, and revise patient care plans based on changing patient needs and clinical findings. Coordinate care to ensure optimal patient outcomes and continuity of services. Clinical Care Delivery Provide high-quality skilled nursing services within the RN scope of practice. Deliver care in accordance with physician orders, agency policies, infection control standards, and regulatory requirements. Manage and monitor complex patient conditions in the home setting. Perform medication reconciliation and ensure safe medication management practices. OASIS & Documentation Complete comprehensive patient assessments, including OASIS documentation, accurately and within required timeframes. Maintain timely, detailed, and compliant clinical documentation in accordance with agency policies and payer requirements. Ensure all visit notes, assessments, and care plan updates are synchronized and submitted promptly. Care Coordination & Supervision Collaborate with physicians, therapists, social workers, caregivers, and other healthcare professionals to coordinate patient care. Directly and indirectly supervise care provided by Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs/LVNs). Provide appropriate guidance, instruction, and oversight to support staff in accordance with state and federal regulations. Conduct required supervisory visits as mandated by agency and regulatory standards. Patient & Family Education Educate patients and caregivers regarding diagnoses, treatments, medications, disease management, and safety measures. Support patients and families in achieving healthcare goals and improving quality of life. Required Qualifications Current, active Massachusetts Registered Nurse (RN) License. Home Health nursing experience. Experience completing OASIS assessments and documentation. Strong clinical assessment and care planning skills. Valid driver's license and reliable transportation. Ability to travel throughout the assigned coverage area. Excellent communication, organization, and documentation skills.
