Contractor - Registered Nurse - OAHMP SMCIL is looking for a Registered Nurse to work part time on a Contractual basis for our Older Adult Home Modification Program (OAHMP) using the CAPABLE program. The ideal candidate must have a Bachelor's degree in Nursing and licensure from the Maryland Board of Nursing. He/She needs the ability to work from a collaborative, client-centered model and the willingness and ability to implement a protocol and balance with clinical judgment. Must have at least one year experience as a licensed Registered Nurse and experience in home care with older adults is preferred. The coverage area will consist of Calvert, Charles, and St. Mary's counties. Must be able to pass a criminal background check. General Job Description of Registered Nurse for CAPABLE The CAPABLE Registered Nurse (RN) will collaborate with older adults to deliver nursing components of the CAPABLE program after completion of required CAPABLE training through Johns Hopkins University School of Nursing. CAPABLE is a home-based program to address functional opportunities and promote independence and tenure in the community with older adults The RN will work with CAPABLE participants to understand their goals, assess and suggest strategies in peoples' homes who have volunteered to participate in CAPABLE. The CAPABLE program consists of up to four RN home visits over 4-5 months. Clinicians meet with participants in their home unless otherwise determined. The RN uses training and motivational interviewing techniques to conduct a comprehensive interview to identify and prioritize issues that matter to the participant. These goals become the foundation for brainstorming and action plan development and implementation. The RN documents the participant's own goals in up to 3 Action Plans. The RN interacts and communicates with the CAPABLE Occupational Therapist (OT) and other team members as needed to ensure close collaboration throughout and after the program. The primary duties and responsibilities of the job: 1. Conduct a comprehensive, semi-structured interview to identify and prioritize issues related to medications, pain, depression, strength and balance, primary care provider communication, and other topics as prioritized by the older adult 2. Engage participants in problem-solving to identify behavioral and environmental contributors that may impact function. Coach and guide the older adult in building self-efficacy and confidence 3. Reinforce brainstorming techniques, strategic thinking, and more independent problem-solving to promote the ability to live at home and in the community 4. Provide education and resources to address future needs and the participant's priorities 5. Communicate with the OT and other team members, including primary care providers 6.
