Job Alerts Position Summary:
The Stroke Coordinator is responsible for supporting the activities focused on improving and reporting stroke care outcomes. Provides outcomes reports and identifies performance and process improvement opportunities. Provides stroke education to staff and community and participates in stroke center research.
Essential Functions and Responsibilities:
Development and monitoring of provision of ongoing educational programs for patients with neurological disorders, including stroke, aneurysm and other neurological conditions as served by the program. Mentoring and support for staff serving in the stroke program, including orientation to stroke policies/procedures, stroke program standards and evidence based medicine with regard to stroke treatment guidelines and best practices. Community based educational programs including EMS carriers and outreach for stroke prevention, at least once per year. Support education of EMR for telemedicine program with network affiliates and staff as indicated. Assist in development and provision of stroke education programs for McLaren Macomb and McLaren Stroke Network affiliates as indicated. Prepares stroke program clinical outcomes reports for metrics that are monitored for the Macomb and the McLaren Telestroke Network. Works with the Stroke Medical Director/ED Director/Stroke Network Directors to develop, implement and utilize appropriate clinical pathways for neuro/stroke patients based on stroke program accreditation standards and evidence based medicine. Reviews compliance of care delivery protocols for all Stroke Network stroke alerts by reviewing documentation and reporting variances, as indicated. Compiles, analyzes and evaluates quality and clinical data to identify patterns or trends; ensures the accuracy and completeness of data used for measuring and reporting physician and stroke program outcomes. Provides real time abstraction of required data elements for patients receiving IV-tPA, mechanical thrombectomy or aneurysmal coiling to provide timing feedback and achievement of Joint Commission and evidence based practice standards of care. Works with external data abstractors regarding fall out cases and assists in optimizing provider documentation to meet standards. Manages process for follow up phone calls to stroke patients after discharge ensuring stroke certification standards are met and analyzes data obtained for dashboard reports and other programmatic needs as indicated. Prepares affiliate dashboard for achievement of stroke care targets, monthly. Participates in stroke center research studies, in coordination with Stroke Center Medical Directors and Institutional Review Board Works collaboratively with Quality Management for cases for review for NeuroScience Quality Improvement Committee for appropriate documentation and communication of these reviews, monthly. Participates in Medical Center and/or departmental quality assessments and continuous quality improvement opportunities, goal setting, collection and evaluation of data, implementation of action plans, or related applications. Performs other duties as required or requested. #LI-KH1
Required:
Bachelor of Science Nursing Licensure to practice in the State of Michigan Three to five years of related clinical experience
Preferred:
Master of Science Nursing, Clinical Nurse Specialist, Advanced Practice Nurse or Nurse Practitioner NIHSS certification
